Instructions for Concept Mapping:
  1. Place primary diagnosis in center of map
  2. Surround with related assessment data and lab values
  3. Show nursing diagnoses connected to primary problem
  4. Use colors to organize: RED=Problems, BLUE=Assessment Data, GREEN=Interventions, YELLOW=Outcomes
  5. Draw lines showing relationships and indicate type of connection
  6. Update as new information emerges
Color Guide:
RED = Patient Problems/Diagnoses
BLUE = Assessment Data, Vital Signs, Lab Values
GREEN = Nursing Interventions
YELLOW = Expected Outcomes
